One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards created by point of operation, ingoing nip points, rotating parts: On or about December 8, 2016 and there prior to, the Employer provided employees operating plastic film extruder(s) with loose over-garments, exposing employees to caught-in hazards.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about December 8, 2016 and there prior, the employer allowed employees to get into the plastic film extruder and adjust moving parts such as but not limited to rollers, spindles, and or winders when the extrusion machine was still running, exposing employees to caught-in hazards.
