29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start-up or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     On or about December 13, 2016, the employer had not developed an energy isolation program for employees required to service and/or perform maintenance on equipment such as but not limited to the Knot machines.  Failure to do so exposed employees to point of operation hazards if the machines were unexpectedly energized.

This condition exposed employees to the hazard of contact with the rotating saw blade.
