29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about December 6, 2017, employees were exposed to the hazard of being caught in the blades because the employer did not ensure that the energy control procedures was periodically inspected to ensure that the procedure was adequate.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  Employees, who were authorized to performed lockout/tagout on the Knot saw blades #1, #2 and #3 and Delta radial arm saw were not provided training by the employer to ensure they understood the type and the magnitude of the energy being controlled and the methods and means necessary for energy isolation and control.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The Ouachita Hardwood Flooring was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection number 1197181, citation number 1, item number 2 and was affirmed as a final order on March 17, 2017, with respect to a workplace located at 501 Pennington Rd. Warran, AR 71671.  On or about December 6, 2017, and times prior to in the finish knot saw area, the cutting blade on Knot saw #1, #2 and #3 were not guarded.  Employees were exposed to the hazard of being caught-in the rotating saw blade.

The Ouachita Hardwood Flooring was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.213(h)(1): which was contained in OSHA inspection number 1197181, citation number 1 item number 3 and was affirmed as a final order on March 17, 2017, with respect to a workplace located at 501 Pennington Rd. Warren, AR 71671.  On or about December 6, 2017, and times prior to, in the Moulder area, the lower portion of the blade on the Delta radial arm saw used to cut wood was not guarded. Employees were exposed to the hazard of being caught-in rotating saw blade beneath the table it was used on.

The Ouachita Hardwood Flooring was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.213(h)(3) which was contained in OSHA inspection number 1197181, citation number 1, item number 4a and was affirmed as a final order on March 17, 2017, with respect to a workplace located at 501 Pennington Rd, Warren, AR 71671.  On or about December 6, 2017, and at times prior thereto, in the molder area, the Delta radial arm saw used to cut wood boards extended beyond the cutting table.  Employees were exposed to the hazard of being caught-in the rotating saw blade.

Warren, AR 71671.  On or about December 6, 2017, and times prior to, in the Moulder area, the Delta radial saw did not return automatically when released.  Employees were exposed to the hazard of being caught-in the rotating saw blade.
