29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Roll forming machine: On or about December 14, 2016, and at times prior thereto, employees engaged in fabricating metal using the roll forming machine were exposed to caught-in hazards, while the machine had a conveyor roller missing a end guard. b) Chop Saw: On or about December 14, 2016, and at times prior thereto, employees operating a chop saw were exposed to struck-by hazards, while the lower exposed portion of the saw was not guarded to the full diameter of the abrasive wheel.
