(2)  Rotary lift, model SP015, serial #CBB95B005, located in bay 41, was not lowered to rest on the            locking mechanism while the lift was raised and in use.              (3)  Challenger lift, model CL10V3, serial #1004-57392, located in bay 42, was not lowered to rest on the             locking mechanism while the lift was raised and in use.
