29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) On or about December 15, 2017, housekeeping employees used corrosive category 1 cleaners in the janitor's room, including but not limited to BZ7576 ZEP Grout Cleaner and Lime-A-Way without a readily accessible eyewash, exposing the employees to eye damage. b) On or about December 15, 2017, kitchen employees use corrosive category 1 cleaners in the dish-washing area, including but not limited to Detergent 3000 without a readily accessible eyewash, exposing the employees to eye damage.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) On or about December 15, 2016, in the kitchen area, a fire extinguisher, hanging on a wall above a mop-sink was not readily accessible to employees in the event of an emergency because it access was obstructed by buckets and mops, exposing employees to smoke inhalation in the case of a fire emergency.
