29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about December 14, 2016, the employer failed to develop specific procedures for the control of hazardous energy during maintenance and servicing activities. Employees perform required duties, such as repairing a hydraulic cylinder on hydraulic presses in "Packaging" area, or cleaning the lint trap on Lavatech dryers.

On or about December 14, 2016, the employer failed to develop a written certification of hazard assessment to verify that the required workplace hazard assessment was performed. Employees have exposure to chemicals such as sodium hydroxide and sulfuric acid in the water treatment process.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: On or about December 14, 2016, the employer had not developed and maintained a written hazard communication program that included a list of hazardous chemicals in the workplace. Sulfuric acid was a hazardous chemical used in water treatment operations.
