29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a. On or about August 5, 2020, and at times prior, in the Cutting Department, both ends of Cutter 11 did not have the point of operation adequately guarded resulting in employees being exposed to amputation and laceration hazards. Employee exposure to the hazard resulted in a finger tip amputation injury on or about July 14, 2020.

On or about July 14, 2020, and at times prior, employees who had occupational exposure to bloodborne pathogens when cleaning up after an amputation injury, were not offered the hepatitis B vaccine.

Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program.  a.
