29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a) On December 16, 2016, at the front of the church, atop the concrete awning, an employee engaged in Christmas decoration activities was not protected by guardrail systems, safety net systems, or personal fall arrest systems.  Employee was exposed to fall hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   (a)  On December 16, 2016 at the front of the church, a Werner 12 foot fiberglass stepladder used to gain access to the concrete awning, did not extend at least 3 feet above the landing, and there was no grab rail at the top of the landing.
