29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: On or about December 14, 2016, at Cable Quest, Inc. on the testing and manufacturing floor: Employees cutting cable were exposed to struck-by hazards from cable sheathing and fibers when they used a Chicago Electric 4.5-inch Portable Band Saw without wearing eye protection.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. On or about December 14, 2016, at Cable Quest, Inc. on the testing and production floor: Employees performing cable testing and manufacturing were exposed to struck-by hazards from falling materials and the powered industrial truck when the employer's training program did not consist of all required training elements.
