29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a.  On or about August 17, 2018 employees were exposed to electrical hazards where a relocatable power tap was attached by cable ties to the work station frame in the custom cable area to supply power to hand tools and an overhead light fixture.    b.  On or about August 17, 2018 employees were exposed to electrical hazards where a relocatable power tap was attached by cable ties to the work station frame in the Wire Prep area to provide power to a cut-strip machine.    c.  On or about August 17, 2018 employees were exposed to electrical hazards where two relocatable power taps were connected in series (daisy chained) at the wire harness table in the Harness Area to provide power to hand tools such as a heat gun.    d.  On or about August 17, 2018 employees were exposed to electrical hazards where a relocatable power tap was attached by screws to the wood frame of a mobile harness easel in the Harness Area to provide power to overhead lighting fixtures.    e.  On or about August 17, 2018 employees were exposed to electrical hazards at complex assembly where a 4 inch by 4 inch pull box intended to be mounted to the building structure was used as a free hanging pendant box in which were installed receptacles for cord and plug connected equipment.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where flexible cable was used to provide power to a 4 inch by 4 inch electrical box mounted above the work station light fixture in the Custom Cable area.  The electrical box and receptacles provided power to the light fixture and to a relocatable power tap.    b.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where flexible cable was used to provide power to a 4 inch by 4 inch electrical box and receptacles attached to a vertical component of the work station frame at the assembly tables in the Wire Prep area.  The box and receptacles were used to provide power via cord and plug to another 4 inch by 4 inch electrical box and receptacles in the Wire Prep area.    c.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where flexible cord and plug were used to provide power to a 4 inch by 4 inch electrical box and receptacles attached to a conduit frame supporting an overhead light fixture in the Wire Prep area.  The receptacles were used to provide power to the overhead light and to a relocatable power tap into which was plugged a cut-strip machine.    d.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where flexible cable was used to provide power to a 4 inch by 4 inch electrical box used as a pendant receptacle at the Complex Assembly station.  Connected to the electrical box was a relocatable power tap, a "Milwaukee" heat gun, and an extension cord connected to a table top light fixture.    e.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where flexible cord and plug were used to provide power from a 4 inch by 4 inch electrical box to a table top light fixture at the Complex Assembly station.

On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where cable ties were used to attach  the electrical cords of an overhead light fixture and a relocatable power tap to the work station frame in the Custom Cable area.    b.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where cable ties were used to attach  the electrical cord of the light fixture to the work station frame at station AA1 in the Custom Cable assembly area.    c.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where cable ties were used to attach  the electrical cord of the light fixture to the work station frame at station AA3 in the Custom Cable assembly area.    d.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where cable ties were used to attach  a yellow flexible cable to the workstation frame to provide power to a 4 inch by 4 inch electrical box and receptacles at the assembly tables in the Wire Prep area.     e.  On or about August 17, 2018 employees were exposed to hazards related to electrical installation methods where cable ties were used to attach a  flexible cord and plug used to provide power from one 4 inch by 4 inch electrical box and receptacles to another 4 inch by 4 inch electrical box and receptacles attached to a conduit frame supporting an overhead light fixture in the Wire Prep area.
