29 CFR 1926.451(g)(1): The employer did not ensure that each employee on a scaffold more than 10 feet (3.1 m) above a lower level was protected from falling to that lower level. This violation occurred on December 21, 2016, where at least one (1) employee was working outside the guardrail system of a scissor lift and exposed to a fall of approximately 15 feet to the concrete below.

29 CFR 1926.760(a)(1): The employer did not ensure that each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. This violation occurred on December 21, 2016 and at times prior thereto when the employer did not ensure that employees working without fall protection were protected from a fall of approximately 16 feet to the concrete below.
