Meadville Forging Company, L.P., 440 McClellan St., Cambridge Springs, PA, Northeast corner exit of Maintenance area, on or about March 15th, 2017, and times prior thereto: One oxygen cylinder and one acetylene cylinder were stored on a cylinder cart directly next to the exit door.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level: a.) Meadville Forging Company, L.P., 440 McClellan St., Cambridge Springs, PA, Northeast exit of CNC Machining area, on or about December 22nd, 2016 and times prior thereto: The Northeast exit door located in the CNC Machining area was obstructed by snow/ice which did not allow the door to be opened in the event of an emergency. b.) Meadville Forging Company, L.P., 440 McClellan St., Cambridge Springs, PA, "Chip removal" exit east side of building, on or about December 22nd, 2016 and times prior thereto: Directly outside of the exit door there was an approximate 6.5 inch curb which created a potential trip/fall hazard along the egress route.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a.) Meadville Forging Company, L.P., 440 McClellan St., Cambridge Springs, PA, Northeast corner of Maintenance area, on or about March 15th, 2017, and times prior thereto: Employees were potentially exposed to a fire/explosion hazard in that, one oxygen cylinder was stored directly next to one acetylene cylinder on a cylinder cart. The valve protection caps were not affixed to either cylinder.

29 CFR 1910.106(e)(2)(iv)(c): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), may be used only where there are no open flames or other sources of ignition within the possible path of vapor travel: a.) Meadville Forging Company, L.P., 440 McClellan St., Cambridge Springs, PA, Pressure washer area, on or about December 22nd, 2017, and times prior thereto: Flammables including, but not limited to, gasoline were stored within 4 feet of potential ignition sources including, but not limited to, a Shark brand pressure washer. Fueling operation of equipment occurred next to the pressure washer.
