Pennsylvania American Water Company DBA Ellwood City Water Treatment Plant, 907 Lundys Lane, Ellwood City, PA - On or about December 22, 2016 and at times prior thereto, an operator required to utilize an MSA GME/P100 tight-fitting, air purifying respirator, during chlorine change-over and delivery operations, had not been provided a medical evaluation to determine if they were medically able to wear such respirator.
