29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment: a) In the Chlorine Storage Room, on or about November 21, 2024 - The employer did not establish written procedures to maintain the on-going integrity of Halogen Valve Eclipse Actuators. b) In the Chlorine Storage Room, on or about November 21, 2024 - The employer did not establish written procedures to maintain the on-going integrity of the Wallace & Tiernan Vacuum Regulators. c) In the Chlorine Storage and Feed Rooms, on or about November 21, 2024 - The employer did not establish written procedures to maintain the on-going integrity of the Wallace & Tiernan Acutec 35 Gas Detection System. d) In the Chlorine Storage and Feed Rooms, on or about November 21, 2024 - The employer did not establish written procedures to maintain the on-going integrity of the chlorine piping. Failure to develop inspection procedures that establish written procedures to maintain the on-going integrity of process equipment can expose employees to corrosive, toxic, and oxidizing hazards.

29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience: a) In the Chlorine Storage Room, on or about November 21, 2024 - The employer did not test cycle at least once per week the Halogen Valve Eclipse Actuator's actuator system. The actuator's Installation, Operations and Maintenance Manual instructs users to test cycle the actuator system at least once per week to reduce the potential for fouling. b) In the Chlorine Storage Room, on or about November 21, 2024 - The employer did not perform periodic cleaning and periodic preventative maintenance of the Wallace & Tiernan Vacuum Regulators consistent with the regulator instruction book. The book instructs users to perform periodic cleaning after 12,000 pounds of gas has been feed to ensure that elements of the regulator is free of contaminants. The book instructs users to perform periodic preventative maintenance to recondition the regulator at one-year intervals. Failure to perform inspections and tests of process equipment consistent with manufacturers' frequency recommendations can expose employees to corrosive, toxic, and oxidizing hazards.
