29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about December 14, 2016 and times prior thereto, at the site located at or near 6801 W. Sunset Ave in Springdale, Arkansas, employees were using pneumatic nail guns when framing a roof without any eye protection. This condition exposed employees to struck-by hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about December 14, 2016 and times prior thereto, at the worksite located at or near 6801 W. Sunset Avenue in Springdale, Arkansas, employees were framing a roof in excess of 6 feet without any fall protection. This condition exposed employees to fall from height hazards.
