29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about December 13, 2016, the employer failed to ensure that effective machine guarding was in place on the Thompson hand press (die cutting machine). Guarding was not in place to protect the employee from the closing of the press. The bottom platen contained an interlocked bump guard, but did not effectively protect the employee. The equipment is operated in continuous mode with single button actuation. b) On or about December 13, 2016, the employer failed to ensure that effective machine guarding was in place on the hand press 23 (die cutting machine). Guarding was not in place to protect the employee from the closing of the press. The bottom platen contained an interlocked bump guard, but did not effectively protect the employee. The equipment is operated in continuous mode with single button actuation. c) On or about December 13, 2016, the employer failed to ensure that the Chandler & Price die press #15 was effectively guarded to prevent employees from coming into contact with the moving parts of the press including but not limited to the platen die cutting area and mechanical arms. No guarding was in place on the equipment. d) On or about December 13, 2016, the employer failed to ensure that the Chandler & Price die press #16 was effectively guarded to prevent employees from coming into contact with the moving parts of the press including but not limited to the platen die cutting area and mechanical arms. No guarding was in place on the equipment. e) On or about December 13, 2016, the employer failed to ensure that the Chandler & Price die press #17 was effectively guarded to prevent employees from coming into contact with the moving parts of the press including but not limited to the platen die cutting area and mechanical arms. No guarding was in place on the equipment. f) On or about December 13, 2016, the employer failed to ensure that the Chandler & Price die press #11 was effectively guarded to prevent employees from coming into contact with the moving parts of the press including but not limited to the platen die cutting area and mechanical arms. No guarding was in place on the equipment. g) On or about December 13, 2016, the employer failed to ensure that the Chandler & Price die press #10 was effectively guarded to prevent employees from coming into contact with the moving parts of the press including but not limited to the platen die cutting area and mechanical arms. No guarding was in place on the equipment. h) On or about December 13, 2016, the employer failed to ensure that effective guarding was in place on the flywheel of the Thompson hand press (die cutting machine). The backside of the flywheel did not contain a barrier guard to protect employees when working on the rear portion of the press. i) On or about December 13, 2016, the employer failed to ensure that effective guarding was in place on the flywheel of hand press #23 (die cutting machine). The backside of the flywheel did not contain a barrier guard to protect employees when working on the rear portion of the press.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On or about December 13, 2016, the employer failed to ensure that chains and sprockets located on the top of hand press #23 (die cutting machine) were effectively guarded to prevent employees from the pinch point. The chains and sprockets of product roll motors were not guarded.
