29 CFR 1926.451(c)(1)(iii): Ties, guys, braces, or outriggers were not installed to prevent the tipping of supported scaffolds in all circumstances where an eccentric load, was applied or was transmitted to the scaffold: (a) Jobsite - 5851 Keystone Drive, Bethel Park, PA: On or about December 28, 2016, guys, ties, braces, or outriggers that support the scaffold from tipping were removed during the wash down process. An eccecntric load from the Shark Pressure Washer in addition to employees body weight was placed to the intended scaffold.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Jobsite - 5851 Keystone Drive, Bethel Park, PA: On or about December 28, 2016, all employees working on scaffolds were not provided with training to recognize and minimize hazards.
