29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about December 29, 2016, the employer failed to ensure that operators of the Linde Powered Industrial Vehicle (Serial # 351J04A4630) were certified as competent to operate by ensuring they completed the required training. The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 726441.015, citation number 3, item number 1, and was affirmed as a final order on February 28, 2013, with respect to the same workplace location of 3422 Navarre Road SW, Canton OH 44706.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section. On or about December 28, 2016, the employer failed to provide effective guarding on machinery to include but not limited to the Paper Tube Lines 1-4, to protect employees from moving parts and ingoing nip points.
