Abatement certification required within 10 days after abatement date. 29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)  6395 SR 103 North Building 62, Lewistown PA -  On or about December 28, 2016 and times prior thereto, the employer failed to establish a Lock out/Tag out program with procedures for the control of energy, periodic inspections and training to ensure employees were protected before starting servicing or maintenance on machines or equipment.  Employees were exposed to caught-in and struck-by hazards from electrical, hydraulic and mechanical energy sources.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
