On or about December 29, 2016, at the above addressed jobsite, employees performing carpentry operations from the openings of the third floor were exposed to falls, of twenty (20) feet zero (0) inches, created by the presence of unprotected sides.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. On or about December 29, 2016, at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold, twenty (20) feet zero (0) inches above the ground, were exposed to injuries from falls due to the lack of guardrails. The Caliseum II Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1015985, citation number 1, item number 5 and was affirmed as a final order on October 14, 2015, with respect to a workplace located at 2647 W. Medill Ave, Chicago, IL 60647.
