The dust collector (United Air Specialist Model SFC 8-2 sn 60071776) included both electrical and pneumatic energy sources. This condition exposed employees to electrical and amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Trustile Doors, LLC, at 1732 E. 66th Ave., Denver, CO 80229: On and before 12/20/2016, the employer did not ensure that machine guarding was provided on the dust collector (United Air Specialist Model SFC 8-2 sn 60071776) for the axis CNC machine to protect operators and other employees from hazards such those created by rotating parts. An employee was leveling out the accumulation of dust in the 55 gallon drum, when his finger was caught in the dust collector's rotary airlock, leading to an amputation. This condition exposed employees to amputation hazards.
