29 CFR  1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.   On or about 2/7/2025, the employer did not develop and/or enforce the use of machine specific procedures or additional methods and instructions for the control of hazardous energy sources such as but not limited to electricity or pneumatic energy. The operator and/other employees working in the immediate area were potentially exposed to flying projectiles and/or shards of the abrasive wheel and other materials if the wheel were to break or material got caught between the frame and work rest.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a) On or about 2/7/2025, finishing spray chemicals used inside the offline spray booth were not properly labeled. The employer did not ensure that chemicals such as but not limited to primers, adhesives, and/or paint thinner removed from the original container and placed in secondary containers were properly identified as to the contents exposing employees to chemical hazards.
