1910.28(b)(3)(i): Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following, covers: a) Dump Cone Platform - On or about January 4, 2017, the hatch built into the work platform surface, which is approximately 30 inches by 28 inches in size located approximately 18.6 feet above a lower level, was in the open position while employees are monitoring dump cone loading operations.
