29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:    a) Jobsite, 201 Durrow Court, Mars, PA:  On or about January 5, 2017, employees who had been previously trained on the use of fall protection did not retain the knowledge or understanding necessary to work safely on a residential roof at elevations of approximately 35-38 feet .  Employees did not maintain 100% fall protection and improperly attached lanyards to the side D rings of their harnesses.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 201 Durrow Court, Mars, PA:  On or about January 5, 2017, employees were exposed to fall hazards while working on a residential roof and the employer did not maintain a written certification record of training.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Jobsite, 201 Durrow Court, Mars, PA:  On or about January 5, 2017, an employee was working on a residential roof with an 8 in 12 pitch and was not protected from falling as he moved from one anchorage point to another, subsequently the employee fell approximately 35-38 feet to the ground below.
