Power House #1 North end, Level 55-  (a) The open sided floor above the Fish Screen Assembly area had a partial toe guard, leaving an unguarded 57" opening, exposing employees to struck-by hazards of falling objects.

Power House #1, Elevation 37 Bay 7-   (a) The Cooling water strainer unit has an unguarded pulley sprocket, located approx. 18 inch off the floor, with an opening of 11 inches, exposing employees to serious injuries from rotating parts.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) The 40 ton Hydraulic Press, SN#5865, Model PSS40, located in Power House #1 machine shop was not guarded to prevent flying parts.   (b) The Dake Hydraulic Press, Model # 908150-4, SN#182308, located in B15B machine shop, was not guarded to prevent flying parts.  (c) The SPX Hydraulic Press, located in the gear repair area, was not guarded to prevent flying parts.  (d) The 40 ton Hydraulic Press, SN#5345, Model PSS40, located in Power House #2 machine shop was not guarded to prevent flying parts.  (e) The horizontal band saw's, model 7020M, SN# 0605028, unused 10" portion of the saw blade was unguarded exposing operators to serious injuries.  The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1056021, notice number 1, item number 3 and was affirmed as a final order on 11/05/2015, with respect to a workplace located at Foster, OR 97345.   The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1056022, notice number 1, item number 2 and was affirmed as a final order on 11/05/2015, with respect to a workplace located at Blue River, OR 97413.   The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1056017, notice number 1, item number 1 and was affirmed as a final order on 11/05/2015, with respect to a workplace located at Mill City, OR 97360.
