29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Worksite located at 3101 E Memorial Rd. Edmond, OK 73013. On or about January 04, 2017, adequate protective systems, such as benching, sloping, or shielding were not used to protect employees working in the trench, exposing employees to engulfment or crushed-by hazards.
