On or about January 4, 2017, workers were framing the nursing home under construction up to approximately 28 feet high using a makeshift wooden trash box. The box was elevated by a rough terrain forklift and was not properly secured to the forks. The trash box was not designed, constructed or loaded for the purpose of its use, exposing employees to a fall hazard.

On or about January 4, 2016, workers were framing the nursing home under construction from an elevated trash box approximately 28 feet high that was not properly attached to the Genie rough terrain forklift to prevent the platform from sliding off, exposing employees to a fall hazard.
