Metal Straightener Machine Line/Metal fixed platform - On or about January 10, 2017, the fixed "crossover" ladder-platform on the straightener line # 4 (machine # 4104) was not provided with standard railings on both open sides. Employees were thereby exposed to slips and falls. 1. From west to east direction - only the left descending side of the platform had a handrail. 2. From east to west direction - only the right descending side of the platform had a handrail.

29 CFR 1910.24(e): Fixed stairs were not installed at angles to the horizontal line between 30 and 50 degress. a. Metal Straightener Machine Line/Fixed metal platform - On or about January 10, 2017, the employer had a platform with stair ways on both sides which did not have angles to the horizontal line between 30 and 50 degrees. Employees were thereby exposed to tripping and fall hazards. 1. East side stairway - had its angle degree from the 3rd step to the platform's floor at approximately 57 degree angle. 2. West side stairway - had its angle degree from 4th stair to the platform' floor at approximately 52 degree angle.

29 CFR 1910.95(i)(4): The employer did not provide training in the use and care of all hearing protectors provided to employees: 1. Straightener Machine Line/Toward the end - On or abour January 18, 2017, a machine operator was exposed to a time weighted average (TWA) sound level of 88.13 decibels, which exceeded an 8-hour time-weighted average sound level of 85 decibels measured on the A scale for the 270 minute sampling. Exposure calculation included a zero increment for the 210 minutes not sampled. 2. Building # 3/End of the Line - On or about January 18, 2017, a machine helper was exposed to a time weighted weighted average (TWA) sound level of 87.76 decibels, which exceeded an 8-hour time-weighted average sound level of 85 decibels measured on the A scale for the 225 minute sampling. Exposure calculation included a zero increment for the 255 minutes not sampled. Yet, the employer's training program did not include adequate training on the proper care and maintenance of hearing protectors such as ear muffs and ear plugs.

Building # 3/Metal Staightener machine Line - On or about January 18, 2017, employees working at the machine line were observed wearing personal protective equipment (ppe) that was dameged. Employees were thereby exposed to the physical hazards of noise. 1. The NS - Cap Mount - Ear muffs had cracks/dents acroos the outer plastic ear cups pieces and the (R) inner piece was broken.
