Paragon Tempered Glass, LLC - Antwerp, Ohio: On or about December 22, 2016, employees who manually handled raw glass were exposed to laceration hazards.  The employer did not ensure that employees were provided with and used personal protective equipment that would protect parts of the body such as, but not limited to, the neck, arms, and legs, from laceration injuries.

29 CFR 1910.132(f)(1)(v): Employee(s) required to use personal protective equipment (PPE) by this section were not trained to know the proper care, maintenance, useful life, and disposal of the PPE:    a. Paragon Tempered Glass, LLC - Antwerp, Ohio: On or about December 22, 2016, the employer did not ensure employees who were required to wear cut resistant gloves and arm guards/sleeves to protect against laceration injury were trained in the care, maintenance, and useful life of the personal protective equipment (PPE).  Employees that handled raw glass used arm guards that were worn and ineffective in that the PPE did not properly stay in place on the arms.

Paragon Tempered Glass, LLC - Antwerp, Ohio: On or about January 9, 2017, the employer did not ensure that employees who service and maintain the Ianua Screener (model S031; s/n 2172/02) were adequately trained in the skills required for the safe application, usage, and removal of energy controls.  Employees that cleaned the silkscreen were not trained to control hazardous energy sources of the machine.

Paragon Tempered Glass, LLC - Antwerp, Ohio: On or about January 9, 2017, employees were exposed to crushing and pinch point hazards in that the Ianua Screener (model S031; s/n 2172/02) was not locked out prior to cleaning under the screen.  The employer did not ensure that energy control application steps to control the machine's electrical, pneumatic, and gravitational energy sources were implemented prior to servicing tasks; the employer did not:    -  Ensure employees had the method or means to control hazardous energy.  -  Ensure that equipment was turned off or shut down.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  -  Verify that isolation and deenergization of the machine or equipment had been accomplished.
