On or about April 27, 2025 at Paragon Tempered Glass, LLC located at 5046 CR 424, Antwerp, Ohio: The employer did not ensure an employee performing servicing on the intermediate roller conveyor located between the washer and the flat furnace, locked out the roller conveyor prior to reaching under the conveyor to make adjustments to a roller conveyor drive belt, exposing themselves to caught -in and crushing hazards from the conveyor's rotating drive shaft.

29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a. On or about April 27, 2025 at Paragon Tempered Glass, LLC located at 5046 CR 424, Antwerp, Ohio: The employer did not ensure an employee performing servicing on the intermediate roller conveyor located between the washer and the flat furnace, was trained as an authorized employee prior to making adjustments to a roller conveyor drive belt, exposing themselves to caught -in and crushing hazards from the conveyor's rotating drive shaft. Training did not include the following information: 1. The recognition of applicable hazardous energy sources, 2. The type and magnitude of the energy available in the workplace, 3. The methods and means necessary for energy isolation and control.
