29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: JOB SITE: #3 Williams Drive, Union, MO 63084, on or about 1/10/2017, in production area, next to the SCMI 240v T-130 Shaper Woodwork Machinery, a 4-way electrical receptacle, designed for fixed installation was wired with a black flexible cord from a Retractable cord reel and was not securely affixed to any surface. The electrical receptacle was used to charge battery operated power tools.

Workers were exposed to an electrical hazard.
