29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  JOB SITE: 435 County Road 6430, Salem, MO 65560, on or about 2/2/2017,  CANOAK Missouri, LLC had not established a program consisting of energy control procedures, employee training and periodic inspections for the Baxley Equipment (wood stacker, graders, and trimmers) and the American Wood Dryers-Dry Kilns.  Workers were exposed to an electrical hazard.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l):  JOB SITE: 435 County Road 6430, Salem, MO 65560, on or about 2/2/2017, CANOAK Missouri, LLC had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.  Multiple employees were operating powered industrial trucks without required training.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of ingoing nip points  and rotating parts:  JOB SITE: 435 County Road 6430, Salem, MO 65560, on or about 2/2/2017, workers in the grading and stacking area were exposed ingoing nip points and rotating parts from the Baxley equipment (grader/stacker).  The ingoing nip points and rotating parts measured approximately 33 inches high from the working platform and were not guarded to protect operators and other employees.
