JOB SITE: 435 County Road 6430, Salem, MO 65560, on or about 2/2/2017, workers in the wood flooring cutting production area, were exposed to sprockets and chains from the Doucet Double End Material Handling conveyor.  The ingoing nip points and rotating parts measured approximately 24 -38 inches from floor level were not guarded.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation guards were not designed and constructed to prevent the operator from having any part of his/ her body in the danger zone during the operating cycle:     JOB SITE: 435 County Road 6430, Salem, MO 65560, on or about 3/9/2021, the employer failed to ensure employees were protected from amputation hazards, associated with inadequate guarding with the 14" Mechanical Up-Cut saw.   The guards failed to prevent  the operator from having any part of his/her body in the danger zone during operating cycle.  The fixed guard was not completely enclosed and  allowed approximately 3.5 inch clearance and access to the blade.  There were multiple Up-Saw in the floor cutting production area:  1. Precision Products Company 14" Mechanical Up-Cut Saw  Unit 1, Model: 14, S/N: 1650, 5HP, 480v 2.  Precision Products Company 14" Mechanical Up-Cut Saw Unit 2, Model: 14, 5HP, 480v 3. Precision Products Company 14" Mechanical Up-Cut Saw Unit 3, Model: 14,  5HP, 480v  CANOAK MISSOURI LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.212(a)(3)(ii), which was contained in OSHA inspection number 1201365, citation number 1, item number 3, issued on 3/7/2017 and was affirmed as a final order on 3/30/2017, with respect to a workplace located at 435 County Road 6430, Salem, MO 65560.
