29 CFR 1926.754(e)(2)(iii): 29 CFR 1926.754.(e)(2)(iii): Metal decking holes and openings were not immediately permanently filled with the equipment or structure needed or intended to fulfill its specific use and strength requirements of paragraph (e)(3) of this section, and/or were not immediately covered:  During installation of decking at a Homegoods Store at 1240 Town & Country Crossing Drive, Town & Country, MO on December 27, 2016, an opening in the decking was not covered or guarded to prevent a fall.  IN THE ALTERNATIVE:  29 CFR 1926.761(c)(3): Where CDZ's (controlled decking zone) were being used, the employer did not assure that each employee was provided training to access the zone:  During installation of decking at a Homegoods Store at 1240 Town & Country Crossing Drive, Town & Country, MO on December 27, 2016, the opening in the decking next to the existing structure was not part of training for accessing the deck as part of a CDZ.       Abatement documentation is required for this violation on how the hazard will be prevented in the future.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  During installation of decking at a Homegoods Store at 1240 Town & Country Crossing Drive, Town & Country, MO on December 27, 2016, a personal fall arrest system was not provided and fall harnesses were not worn.  IN THE ALTERNATIVE:  29 CFR 1926.760(c)(3): In each controlled decking zone (CDZ), the boundaries were not designated and clearly marked.  During installation of decking at a Homegoods Store at 1240 Town & Country Crossing Drive, Town & Country, MO on December 27, 2016, a controlled decking zone had not been established, including barriers or lines placed demarcating a CDZ.  Abatement documentation is required for this violation on how the hazard will be prevented in the future.

The injury occurred at a construction site at 1240 Town & Country Crossing Drive, Town and Country, MO.
