29 CFR 1926.453(b)(2)(iv): Employees did not always stand firmly on the floor of the basket and climbed on the edge of the basket or used planks, ladders, or other devices for a work position. On or about 8/20/2018, Ironmasters Steel LLC was working on a building located at 2000 West Clay in St. Charles, MO. An employee was welding the steel structure while working from a Genie Aerial Lift, Model S-45/Serial #1000628. The employee was not standing on the floor of the basket. The employee was observed climbing the guardrails and welding while standing on the mid and top rails of the basket.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about 8/20/2018, Ironmasters Steel LLC was working on a building located at 2000 West Clay in St. Charles, MO. An employee was welding the steel structure while working from a Genie Aerial Lift, Model S-45/Serial #1000628. The employee was not wearing body belt with a lanyard or personal fall arrest system that was attached to the boom or basket while working from the aerial lift.
