29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program: On or about January 10, 2017: a) Integrity Foods, Inc., Athens, GA - On the cook side, an employee working in the freezer room Pack Out was exposed to a noise time-weighted average (TWA) of 88.5 dBA, exceeding the action level of 85 dBA. The exposure was obtained from a sample taken over 428 minutes. Zero exposure for the unsampled period. A training program was not established. b) Integrity Foods, Inc., Athens, GA - On the cook side, an employee working on the Formax Line near the oven was exposed to a noise time-weighted average (TWA) of 90.1 dBA, exceeding the action level of 85 dBA. The exposure was obtained from a sample taken over 416 minutes. Zero exposure for the unsampled period. A training program was not established. c) Integrity Foods, Inc., Athens, GA - On the raw side, an employee working on the Grind Line was exposed to a noise time-weighted average (TWA) of 99 dBA, exceeding the action level of 85 dBA. The exposure was obtained from a sample taken over 462 minutes. Zero exposure for the unsampled period. A training program was not established. d) Integrity Foods, Inc., Athens, GA - On the cook side, an employee working on the Formax Line #2 was exposed to a noise time-weighted average (TWA) of 89.9 dBA, exceeding the action level of 85 dBA. The exposure was obtained from a sample taken over 470 minutes. Zero exposure for the unsampled period. A training program was not established. The Integrity Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.95(k)(1), which was contained in OSHA inspection number 1080771, citation number 1, item number 1c and was affirmed as a final order on July 25, 2016, with respect to a workplace located at 525 Old Hull Road, Athens, GA 30601.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about January 4, 2017: Employees in the Production area were exposed to chemical such as antimicrobial solutions, nitrogen, and carbon dioxide. The employer failed to ensure employees were provided GHS (Globally Harmonized System) information and training on chemical hazards present in their workplace. The Integrity Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 1080771, citation number 1, item number 2c and was affirmed as a final order on July 25, 2016, with respect to a workplace located at 525 Old Hull Road, Athens, GA 30601.
