29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite - The employer did not ensure that portable self-supporting ladders were set with the side rails at least three (3) feet above the upper landing surface exposing subcontractor employees to fall hazards of approximately ten (10) feet. Observed on or about 08 DEC 2016.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall: a) Jobsite - The employer did not ensure that subcontractor employees had a means of transporting soil-filled containers out of an excavation to an upper level in a manner that allowed them to maintain ladder balance. Employees were carrying 55 gallon trashcans up the ladder exposing them to fall hazards of approximately ten (10) feet. Observed on or about 08 DEC 2016.
