29 CFR 1910.253(e)(3)(ii)(C)(3): Flashback protection was not provided by an approved device that would prevent flame from passing into the fuel gas system:  a)Yard area at ELG Metals, Inc.:  On or about January 12, 2017 and at times prior, the exposing employer, Mega MS LTD dba Mega Maintenance exposed employees to fire hazards in that a flashback protector was not in place on the propane gas manifold used in the 5 foot victor cutting torch operation to cut up stainless steel scrap metal.

29 CFR 1910.1026(i)(4)(ii): Employees were allowed to consume food or beverages at a worksite where chromium (VI) was present; however, the employer did not ensure that employees did not enter the eating and drinking areas with chromium (VI)-contaminated protective clothing or equipment:  a)ELG Metals lunch room: On or about January 12, 2017 and at times prior, the employer allowed employees to enter eating and drinking areas with protective clothing and/or equipment on that had not had surface hexavalent chromium removed by a method that didn't disperse hexavalent chromium into the air/or onto the employees body.  Air monitoring of the employee resulted in levels twice the Cr(VI) Permissible Exposure Limit.
