29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment: On or about 01/11/2017, at the above addressed worksite, an employee did not have hand protection exposing him to an amputation hazard from contact with a circular saw blade.

29 CFR 1926.605(b)(2): Unless employees can step safely to or from the wharf, float, barge, or river towboat, either a ramp, meeting the requirements of paragraph (b)(1) of this section, or a safe walkway, shall be provided: On or about 01/11/2017, at the above addressed worksite, employees working on a barge were exposed to a fall hazard while using a ramp from the shore to the barge that did not have guardrails and was not secured to the dock or the barge. Shoreline Foundation Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.605(b)(2), which was contained in OSHA inspection number 1021105, citation number 01, item number 06, and was affirmed as a final order on 01/29/2016, with respect to a workplace located at 1 East 11 Street, Slip #3, Riviera Beach, Florida 33404.
