JOB SITE: In front of 1401 Lake St Louis Boulevard, Lake St Louis, MO 63367, on or about 1/11/2017, two workers were observed on a JLG Aerial Lift, Sunbelt Rental Unit #550043, with one of the workers, wearing PFAS improperly.  The worker donned on the PFAS without tying the legging portion of the PFAS.  The worker was exposed to a fall hazard.
