29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a)  On or about January 12, 2017, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing blade changes on the band saw and whirlwind jump saw.  Failure to lock out the energy sources while these tasks are being performed exposes employees to amputation injuries.

29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of the up cut saw.     a)  On or about January 20, 2016, the employer did not ensure that employees operating the Whirlwind Up Cut Saw were protected from contact with the machines blade.  The employee's hands were within several inches of the machines blade while operating the saw exposing them to amputation injuries.
