29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative controls were not utilized: a) On or about January 19, 2017, the employer failed to implement administrative controls to control employees exposure to occupational noise. An employee working in the production area using the "notcher" was overexposed to noise at an 8 hour time weighted average of 93.30 dBA, which exceeded the action limit of 85 dBA and the OSHA permissible exposure limit of 90 dBA.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program: 1.) On or about January 19, 2017 the employer failed to implement a training program and train employees who are exposed to noise at or above an eight hour time weighted average of 85 dBA. Personal noise monitoring by OSHA revealed that an employee was exposed to an eight hour time weighted average of 93.30 dBA.
