29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about January, 12, 2016: a) Employees were exposed to fall hazards of approximately 15.7 feet while using a front end attachment that was not factory installed. The front end attachment in use was used to transport employees from the ground to the work area on the roofing system.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about January 12, 2017: a) Employees were exposed to struck by hazards when the employer failed to ensure that the forklift operator was properly trained in the operation and function of the forklift in use at the jobsite. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
