29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about January 12, 2017, at the Sawmill's Edger Unscramble Conveyor: The employer allowed maintenance and clean-up activities to be conducted without utilization of clear equipment specific Lockout Tagout procedures to be used, which resulted in the near amputation injury of an employees first digit to the ring finger of the right hand. b. On or about January 19, 2017, and all times prior, at the Sawmill's Edger Unscramble Conveyor: The employer allowed maintenance and clean-up activities to be conducted without utilization of the clear equipment specific Lockout Tagout procedures to be used which exposed employees to amputations and other injuries related to the inadvertent startup and/or movement of the equipment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a. On or about January 18, 2017, at the facility, the employer had never conducted an annual or more frequent inspection of their existing energy control procedures for all LOTO machinery, thereby exposing employees to caught-in, struck-by and electrical shock hazards.
