29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to the work environment to control or eliminate any hazards or other exposure to illness or injury: a) 2nd floor, Ice World: On January 18, 2017 and at times prior thereto an employee was working from an eight foot ladder while re-installing energized fluorescent light fixtures in a ceiling grid. The employee contacted a 277 volt jacketed wire (whip). The employer did not ensure that the employee had been provided with effective instruction and consistent enforcement of safe work practices to enable the employee to recognize and acknowledge potential hazards of the environment and of working on or near energized conductors. Note) Certification and documentation of corrective action ARE required for this item.

29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there was an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees: a) 2nd floor, Ice World: On January 18, 2017 and at times prior thereto an employee was re-installing fluorescent light fixtures in a ceiling grid. Wire nuts and electrical tape were in place on 277 volt jacketed wires. The employee was not protected from contact with the energized wires by other personal protective equipment (gloves). Note) Certification and documentation of corrective actions ARE required for this item.

29 CFR 1926.416(a)(1): Employees were permitted to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) 2nd floor, Ice World: On January 18, 2017 and at times prior thereto an employee was re-installing fluorescent light fixtures in a ceiling grid. The employee contacted jacketed wires (whips). The "whips" were energized with 277 volts, and had wire nuts secured with electrical tape at the ends. Note) Certification of corrective and documentation action ARE required for this item.
