29 CFR 1926.95(c): All personal protective equipment was not of safe design and construction for the work to be performed: a) Production floor, Centennial Plastics: On January 30, 2017 and at times prior thereto an employee was performing activities in support of a new switch gear installation. The employee removed a vent panel and worked inside of the metal case for the 480 volt energized buss. The gloves worn by the employee did not provide adequate protection for potential hazards, including but not limited to an arc flash. Note) Certification of corrective actions IS required for this item.

29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, were as the employee was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) Production floor, Centennial Plastics: On January 30, 2017 and at times prior thereto an employee performed work above a 480 volt energized switch gear assembly. The employee was feeding metal fish tape into a three and a half inch diameter pipe approximately 36 inches above the energized buss. An effective means of insulation or other safe work practice was not used to protect the employee from accidental contact, direct or indirect, with the energized buss and the hazard of an arc flash. Note) Certification of corrective action IS required for this item.

29 CFR 1926.416(a)(3): Before work had begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit: a) Production floor, Centennial Plastics: On January 30, 2017 and at times prior thereto an employee performed work above a 480 volt energized switch gear assembly. The employee removed a six inch bolted cover panel from the metal enclosed switch gear. Effective safe work practices, including but not limited to, a hazard assessment, was not used to determine if direct or indirect physical contact could be made with the energized buss and the tools being used or by another foreign object. Note) Certification of corrective action IS required for this item.
