29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about January 19, 2016, an employee was not protected from a fall while walking/ working from the second floor of a commercial building, exposing the employee to a fall of 17.8 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: On or about January 19, 2017, an employee was exposed to 17.8 foot fall hazard while using a fall protection anchor that was not installed per the manufacturer's specifications and would not support 5,000 pounds. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
