29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces: Press Area: Stairways were not provided over the inclined conveyors next to the extractors. A worker was injured when attempting to step over a conveyor and fell and his feet went under the bell of the extractor. The extractor bell descended and injured an employee's feet.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Press Dept.: The exit end of the extractor was not guarded while the bell of the extractor was descending. A worker was injured while climbing over the inclined conveyor to clear a jam.
