29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: Instruction Classroom 202: A US Motors polyphase general purpose motor connected to Panel D #1,3,5, used for instructional purposes, had a notched shaft that was 2 inches long, the diameter was less than 1 inch and was not adequately guarded.

29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 and did not make it available for inspection by the Assistant Secretary of Labor and his or her authorized representatives: HVAC Program, Second Floor, Upper Building: Employer had not documented procedures that address electrical safety hazards when employees provide instruction to students in servicing and maintenance of various equipment such as motors.
